Lovren was not at fault for the first. He correctly played him offside. Unfortunately, he wasn't to know that Gomez was slow moving and played him on. Lovren held the line. Gomez was at fault.
You're right. Lovren isn't even close to "100%" at fault for this goal. Gomez is for the majority of it and Mignolet for coming out half arsed and missing it, which he shouldn't have to do in the first place had Gomez just held the line.
Oh and stepping up to play offside is a perfectly legitimate and indeed essential part of a defensive handbook.....suggesting otherwise is ludicrous.

Matip is clearly playing the offside trap too. After the ball is played over the top, he even raises his hand for offside because everyone in front of him is on the right line, playing Kane offside. It's only Gomez, who is behind him and who is not on the right line, playing him on.

Matip drops back, checks the line, and stays right level / in front of Lovren, you can even see him checking the line in the picture DanMann put up. In that situation, Lovren is in charge and the rest of the defensive line should be holding his line. Everyone does so, bar Gomez who has no business at all sitting deeper than any of the two CB's to begin with. It's not the first time where Gomez has completely messed up the offside trap (clearest example being one of the goals against Leicester where the whole team was moving forward and he alone took a step back.)
Whether it is or isn't the right decision to play the offside trap there is up for debate, but it's a perfectly viable option and had Gomez not switched off yet again like he has done in similar situations before, we wouldn't be talking about it.
